Join us on Zoom Sept 18th at 4 pm for a presentation about how a geriatric care manager can help you plan for successful aging. The panelists are experts in aging and work in the field of geriatric care or case management. In their role, they can work with their clients to plan for budget friendly aging. Our speakers will share their expertise including when it is best to engage with them, and what this partnership can do to help you plan for your future.
Our panelists include:
- Jenny Miller-Koehler, certified care manager, social worker and business owner of Senior Care Concepts,
- Hillary Leduc, a case manager, gerontologist, and a certified Dementia Practitioner and Assistant Director of Elder Rights and Safety and Case Management Coordinator for the Office of Healthy Aging, and
- Jamie Sanford, a navigator, Licensed Clinical Social Worker (LCSW), Licensed Nursing Home Administrator (LNHA), and Licensed Assisted Living Administrator in the State of Rhode Island.

Rebecca Wild-Wesley, retired RN, a gerontologist, and certified care manager and co-chair of the Health & Wellness committee.

- Hillary Leduc is a dedicated professional in the field of Aging and Gerontology, currently serving as the Assistant Administrator of Elder Rights and Safety and Case Management Coordinator for the Office of Healthy Aging. With 12 years of diverse experience across Assisted Living, Nursing Homes, Private Home Care, and Case Management, Hillary brings a wealth of knowledge and compassionate care to her work. Hillary holds a Bachelor’s degree in Sociology with a Minor in Social Work from Rivier University and a Master’s degree in Gerontology from Concordia University. She is also a Certified Dementia Practitioner, recognized by the National Council of Certified Dementia Practitioners. The Office of Healthy Aging oversees four Case Management agencies who provide case management to individuals enrolled in our At Home Supports Program, Adult Protective Services and Family Caregiver Programs. These programs support low income, at risk elders and caregivers in Rhode Island and provide them with supports and services to age strong and while keeping in mind their right to self determination.
- Jenny Miller Koehler, MSW, CMC is the Founder and Owner of Senior Care Concepts Inc. She has been a social worker for 27 years, having previously worked at local assisted living communities in social work and community relations, learning how best to accommodate seniors with varying levels of need, as well as their families. Jenny’s experience in senior care planning and management has given her extensive knowledge of the many assisted living and rehabilitation facilities in the state, access to dozens of physicians and specialists, and connections with a variety of at home service providers. During her time at Senior Care Concepts Inc. she has been providing in-depth care assessments and long-term health care planning for seniors, acting as a liaison between health care professionals, seniors and their families. - Jamie Sanford is a Licensed Clinical Social Worker (LCSW), Licensed Nursing Home Administrator (LNHA), and Licensed Assisted Living Administrator in the State of Rhode Island. She currently operates The Loft at Linn, as afford assisted living memory care, Aldersbridge Physical Therapy & Wellness offering outpatient physical, occupation and speech therapy, and the Aldersbridge Navigator offering care management services for older adults located in East Providence RI. Jamie has over 18 years of experience in a variety of settings including community case management for individuals living with developmental disabilities, long term care, hospitals, hospice, nursing home administration, and assisted living memory care.
